Job Requirements/Description
Deliver hospital-level care where it can make a real difference: in a patient’s own home.
RUH Hospital at Home (RUH@Home) is our consultant-led virtual ward, supporting patients who would otherwise need to be admitted to hospital, or who can safely return home earlier with the right clinical support around them. The service works closely with primary, community and social care partners to ensure safe transitions of care.
You will join a multidisciplinary team of doctors, nurses, therapists, therapy assistants and healthcare assistants providing personalised acute care in patients’ homes, seven days a week between 08:00 and 18:00.
In this key role you will be responsible for the assessment, planning, delivery and evaluation of care for patients with acute and complex needs within their own homes. Working autonomously, you will provide expert clinical decision-making, manage clinical risk and support the recognition and escalation of patient deterioration.
As a senior member of our multidisciplinary team, you will coordinate the day-to-day delivery of the nursing service, support patient flow and provide clinical leadership to other staff. You will contribute to service development, quality improvement and the delivery of safe, effective and compassionate care within the virtual ward model.

Support the management of complex and deteriorating patients, ensuring timely escalation and transfer where required.
Work closely with acute, community and primary care partners to support admission avoidance and safe, timely discharge.
Identify and escalate clinical or operational pressures and contribute to service improvement.
Support the effective use of remote monitoring and virtual ward technology.
